Office was dirty (overflowing open trash can in patient exam room, debris on patient exam room and restroom floors). Doctor prescribed a medication I was allergic to, without discussing medication allergies with me or looking at the paperwork I'd filled out that listed my medication allergies. Lack of confidentiality (open exam room doors, allowing me to hear conversations with other patients and his conversations with me to be potentially overheard by others).
