Beware of health notes and inaccurate information
I saw this doctor for a condition I was diagnosed with a few years ago recently. While I was initially happy with the experience at my visit and the thorough discussion about my health concerns pertaining to this particular condition, after the visit and the 'progress notes' and 'other conditions' on the after visit charting on MyChart was almost shocking to me but I wonder now if this is why she had to bring a 'chaperone' with her. To place false information in a health record from outside providers without consulting me is unacceptable, especially considering none of these 'conditions' I was being treated for by any doctor, and had nothing to do with why I scheduled an appointment with Hematology. To claim you are not responsible for what you allow to go into my progress notes when none of that was discussed with me at all also doesn't sit right with me. All of this MyChart exchange from other doctors without consent of patients is invasive and intrusive, and also stigmatizing.
