Was unable to complete a surgery on my family member. Her notes were not descriptive enough and no images to explain what she saw when she opened her up. Waited weeks until we got there to tell her she needs to go to a University. The University started chemo again based on her incomplete...

I consulted Dr. Majka ten years ago, and she diagnosed me as having rheumatoid arthritis. I was in constant, terrible pain; merely putting a shirt on in the morning caused me great anguish. Didn’t do any type of hands on assessment for a new patient physical. Didn’t order meds which was the whole purpose of the visit. Had to do a tele health visit shortly after so he would order my meds and then didn’t get it right.
