Biography: Jeffrey Rayborn, MD is a graduate of The Ohio State University and the University of Cincinnati College of Medicine. He completed his residency in Family Practice and his fellowship in Sports Medicine at Bethesda North Hospital. Dr. Rayborn joined Premier Orthopedics in 2009. Dr. Rayborn's practice focuses on non-operative orthopaedic care for adolescents and adults, and primary care sports medicine. Dr. Rayborn also employs the use of musculoskeletal ultrasound in the evaluation of orthopaedic conditions, as well as in the placement of needles to support the aspiration or injection of joints such as the shoulder, elbow, hip, and knee. The use of musculoskeletal ultrasound has been shown to enhance patient management and outcomes in both general orthopaedic and sports medicine populations. Dr. Rayborn also offers Platelet Rich Plasma Injections (PRP) when medically appropriate. Dr. Rayborn is actively engaged in the sports medicine community. He currently serves as the Medical Director for Upper Valley Sports Medicine Center and is also the team physician at Tipp City High School. Dr. Rayborn is a member of the American Medical Society for Sports Medicine and the American Institute of Ultrasound in Medicine. He maintains office hours at Miami Valley Hospital North in Englewood and the Hyatt Center in Tipp City. Biography: Indresh Venkatarayappa, MD, earned his Doctor of Medicine degree from Bangalore Medical College (India) and completed an orthopedic surgery residency T.N. Medical College (India). He also completed an orthopedic surgery internship and residency at Wright State University Boonshoft School of Medicine. Dr. Venkatarayappa completed surgical training in the United Kingdom at MRCS Edinburgh and was fellowship trained in traumatology at Wright State University Boonshoft School of Medicine and MRCS. He is board eligible in orthopedic surgery. Dr. Venkatarayappa specializes in orthopedic surgery. He is a member of the Member of Royal College of Surgeons, Edinburgh, UK, a Candidate member of American Academy of Orthopaedic Surgeons, and a Candidate member MID - America.

Biography: Robert B. Harrington, MD is a graduate of the University of Dayton and The Ohio State University College of Medicine. He completed his residency in Family Medicine at Miami Valley Hospital in Dayton. Dr. Harrington is the team physician for Wayne High School. He has been in private practice in Dayton since 1999, and joined Premier Physician Network in January 2010. Dr. Harrington is a Credentialed ImPACT® Consultant trained in treating concussions as well as administering and interpreting the ImPACT® test.

Biography: Ariana Herbert, MD, received her medical degree at The Ohio State University School of Medicine, and completed her residency at Wright State University. Dr. Herbert has served as a patient care and nursing assistant and has volunteered in rehabilitative care. Dr. Herbert is board certified in family medicine.

Biography: Dr. Agarwal is a graduate of Jiwaji University in Madhya Pradesh, India. She completed residencies in pathology and internalmedicine at Miami Valley Hospital and Wright State University. Dr. Agarwal is board certified in internalmedicine. She has more than 20 years of experience as a family practitioner.

Biography: Geetha Ambalavanan, MD, FAAFP is a board certified family physician providing comprehensive primary health care to patients of all ages, emphasizing disease prevention. Her special interests includes lipid management, smoking cessation and sleep disorders, in addition to chronic disease management. She has been with Premier Physician Network since 2007.
