Biography: Jacob Van Fleet, MD, is a board-certified family medicine physician who partners with his patients to provide them with high-quality, broad-spectrum care. He strives to strengthen the health of the community by caring for those in need. From chronic disease to basic office procedures, from pediatrics to geriatrics, he hopes to build partnerships with individuals in all facets of life. He enjoys getting to know his patients as individuals and families, and he encourages everyone to champion their own health. _x000D_ Dr. Van Fleet is certified for Medication Assisted Therapy and has completed the University of Arizona Integrative Medicine in Residency Online Program. He is a member of the American Academy of Family Physicians, American Medical Association, American Society for Addiction Medicine and Kitsap County Medical Society. _x000D_ Outside of work, Dr. Van Fleet enjoys piano, hiking and intramural sports.

Other special medical interests include mind and body medicine, adolescent health, and Lesbian, Gay, Bisexual, Transgender, Queer/Questioning (LGBTQ) care. _x000D_ Dr. Tomberg strives to tap into the strengths of each individual, which he believes can help them to live a healthier and happier life. This involves listening to a patient's story and allowing the patient to learn something very important about themselves during the process. He especially enjoys working with adolescents to forge healthy habits for a lifetime. _x000D_ Away from work, Dr. Tomberg enjoys skiing, meditation, singing and general family antics.

Her focus is in primary care, with a special medical interest in working with older patients to enhance and maintain independence and quality of life.As an internist, Dr. Lopata is trained to prevent, diagnose and treat chronic disease. She partners with her patients to manage a range of chronic conditions, such as high blood pressure, high cholesterol, diabetes, thyroid disease, kidney disease and more. She collaborates with specialists when necessary to deliver the highest-quality medical care to her patients.When she is not working, Dr. Lopata enjoys running, traveling, and playing with her dogs.

Care Philosophy: Christopher Quarles, MD, FAAFP, is a board-certified family medicine physician who cares for patients from infancy through geriatrics. Dr. Quarles strives to deliver quality care with an emphasis on preventive health and continuity of care. He partners with his patients to improve their health and the health of their families. Dr. Quarles is a member of the American Academy of Family Practice, American Medical Association and American Association for Physician Leadership. He served 27 years with the U.S. Navy, including two years as commanding officer at Naval Hospital Bremerton. Outside of work, he enjoys traveling, running and reading.

Care Philosophy: My medical philosophy of patient care is based on extensive information gathering at the initial visit and establishing rapport with the patient, followed by discussion of goals for better health. Subsequent visits build up on that foundation of understanding to fine tune their individual treatment plan. My hope is to empower patients to achieve maximal health and quality of life.
