I just found out today, Jan 4, 2016, that Teresa Stephens put false information in my medical file, without my knowledge, back in 2013. I have officially filed a complaint with their office and I am having that false information removed from my medical records. Teresa Stephens should be fired immediately and needs to face the consequences of someone that places false information in someone's medical file.
