Incorrect records

Melody put down the wrong procedure in her notes. When I came in for my follow up she didn't remember anything about the previous appointment and admitted she took bad notes during my last one. When I corrected her what she did wrong she didn't correct it in my records and instead double down on the incorrect notes. Now the office I was seeing her at closed and I can't get the records fixed and I'm being charged for the incorrect things by the billing department and my insurance keeps refusing it.
