Leilani Otero, PA-C

I was the Healthcare Surrogate and caregiver for my 92-year-old mother, who recently passed. She had been seeing Leilani Otero Phelan, PA, of Proactive Primary Care (PPC) in Westchase. My mother suffered from many health issues and was in an assisted living facility (ALF.) Although everyone seemed friendly at PPC, the outcome of their care was poor. The ALF and PPC deal with a lot of elderly folks. As a result of Leilani’s failure to properly follow protocol, the ALF was constantly calling her office to rewrite prescription orders and implied PPC did not know what they were doing. Unfortunately, my poor Mother’s health was in decline. Apparently, Mom seemed to be too much work, so they sent us a termination letter without warning, effective immediately! I was forced to scramble to find another doctor to continue her care, at a time when continuity and consistency was critical. In my opinion, you should avoid PPC or you might experience the same cruel treatment we endured.
