I switched PCPs after she refused to sign a waiver form for a NON-impact over 55 chair exercise class I wanted to attend with my 84-year-old, legally blind father (who lost his wife recently). His doctor signed his form right away. What I got was delay, excuse, delay, excuse...until I was finally told she wouldn't sign it at all- days before the beginning of the class.
