Dr. Deborah Collins, PSY.D

Dr. Collins did not record all of the previous medical records that she read for her evaluation. She did not document important memory deficits that were apparent during her clinical interview and from other medical records she read. . During the clinical interview, she used tutorials to teach a person who she was only supposed to assess for current competency. Her role was not to try to teach the patient information that they didn't know or would not be able to remember a couple of hours after they were done with their clinical discussion. She had mistakes on dates and ages that the person was during historical information and completely left off major documented deficits in executive functioning, working memory, and problem solving. Her report did not have a logical flow and jumped around with information and this made it more challenging to read. Her opinion was slanted towards a couple of positive attributes and not seeing the overall picture.
