Falsified records
I had the displeasure of being seen by this provider twice. In both appointments she was condescending, rude, and had a bad attitude like seeing patients was a chore. She put false information in my medical record without any proof to back up her claims. This false info was then sent to a pain management clinic who called me and treated me as a drug seeker! Upon request of my medical record with Wise Health Systems, I found the false statement no less than 3 times. I completed and turned in the form to have my record amended, but was rejected. I definitely DO NOT recommend seeing this provider.
