You failed them
Dr was aware of all psychological problems with one of the parents. Met with me only one time before deciding to re-unify my kids with the other parent. You were wrong. One of the kids is still living this situation. The other chose no contact with the other parent. You did not talk to the other parent's family who were all concerned as well and still are to this day. You failed my children and ruined their childhood. They are both in therapy today dealing with the same issues that you could have prevented. Shame on you.
