My surgery was done in May of 2023 by Jeff Kozak. Worth noting, I was in excellent health before going to him except for some hip and back pain. "No problem, we can fix that", he told me. The hospital stay was supposed to be 2-3 days......and one surgery. In the first surgery, Dr Kozak nicked the dura of my spinal column. After a week of me having horrible pain and headaches, he performed a second surgery to repair the damage that he had done to my dura (blood clot and spinal fluid leakage at base of my spine.) A few days after the second surgery, he came to my room and convinced me and my husband that I would 'NEVER' have to have another surgery IF I just let him do one more surgery from the anterior. I was having no pain other than a bit of surgical pain at that time, which was normal. I later discovered in the hospital notes, that he had DOCUMENTED that he performed that 3rd surgery b/c I was having pain. This was a blatant lie.... and HE knew it. He told me that I needed the surgery so that I would not need to have surgery in the future.. never a discussion of pain by me or him He, along with Dr Bahtia (Kozak's Vascular surgeon) performed the 3rd and disastrous surgery. I was left with internal bleeding, clots that formed all over my body, including my femoral arteries (R&L), clots in my R iliac artery, clots in my vena cava, in my lung, a clot in my L abdomen the size of a cantaloupe, etc, etc. I demanded to be transferred to the vascular hospital nearby b/c, at this time I had no pedal pulse in my R foot and my R leg was swollen to 3 times its normal size and I had very little feeling in that leg and foot. I needed a vascular surgeon. Dr Kozak HIGHLY resisted my transfer but with my demands, I was transferred. I was told by my vascular surgeon in the new hospital, "You didn't have this before your surgery....you have it now. This happened IN surgery". He said it was caused by an over manipulation of my internal organs during surgery. I had to have 3 more procedures in the Vascular hospital including iliac stents and pulmonary filter which saved my R leg and my life. After 28 days in the hospital and 5-6 units of blood, I went home in a wheelchair. 20 months later, I still suffer from serious neuropathy/pain in my R leg, as well as multiple other physical problems as a result of what Kozak did. I have spent every single day since going home from the hospital doing PT to try to achieve some sort of normalcy in my physical self. I suffer every single day. This man RUINED my life and I certainly will die many years sooner because of what he did to me. The original pain in my hip and back are still there BUT to a much greater degree. I would NEVER, EVER have this man do surgery on anyone who I love, EVER. If I could give negative stars to this man's review, I surely would. Sound like a horror story? It was one and it continues every day.

Wonderful Doctor!
I’ve been a patient of Dr. Kozak’s since 2011 and he is a very conservative Doctor on his treatment. Back then he tried to steer me away from surgery but after 11 years the pain from my back finally reached the levels he mentioned where and when it would get to. Two weeks ago He performed a fusion on my L4&5 and within hours of me being awake after surgery I was pain free! He is a very kind and knowledgeable spine surgeon! Thank you Dr Kozak, his Nurse Bobby and the rest of his amazing staff for giving me back my reason to want to live life to the fullest again!!!!!

Last surgery I had with him , ruins my back. Also the vascular surgeon he used , destroyed my pelvic reconstruction surgery I had in 2016. Then tried to say it was because I had children. Everything was fine before he hired a incompetent surgeon . It’s time he retires

Had a Laminotomy and Microdiscectomy with Dr Kozak in December. His manner and style is through and professional. His PA Billy responded to my questions post surgery quickly. Staff at Texas Orthopedic Hospital was efficient and caring. Thanks to RN Selma pre-op for taking care of me. She has a great touch getting IV started. I have all ready recommended Dr Kozak and Texas Orthopedic Hospital three times since my surgery. I consulted with three surgeons on my illness, I chose Dr Kozak to operate on me.

Had L4 laminectomy and C5 neck fusion 4 years apart. In each case, he did a great job of explaining how it would go before and during, and what to do after, surgery. Have had complete recovery in both cases.

Dr Kozak is an amazing, caring, talented, WONDERFUL man! After dealing with incompetent, didn't give a hoot doctors, I felt so relieved within moments of meeting him! One week out from having Dr K and his team "fix" me (Cervical Spinal Fusion) I am ecstatic at the instant relief from 10 years of pain. The entire team from day one was/is truly concerned with their patient's welfare& care. DR KOZAK IS AMAZZZZING!!!

Dr. Kozak is an incredible Surgeon! He is takes pride in his work, is passionate about what he does and cares about his patients. He is on schedule, listens carefully, answers your questions, makes his diagnosis and prescribes. His skill as s surgeon is remarkable. He performed a three level fusion to my cervical vertebrae. I have had zero pain in two years! Dr. Kozak and his P.A. Billy are an effective and an efficient team. If you have a question and phone, Billy will promptly phone you back,

in 1989 i seen DR Kozak with a broken back he did a interiar lumbar fuson in 1990 ,1991, a extearer lumbar fuson and if i had it all over to do again i would not use any one BUT Dr.KOZAK 28 years has went by with very little problems now i am a 61 year old man with very very little pain in my back i may add i went back to work after i was realised and have worked all this time!!! i never got the chance to tell you Dr. KOZAK THANK YOU FOR MY LIFE BACK!!! Dr. Kozak is very highly recommended for a reason. Thanks to him I have my life back after undergoing a laminectomy/microdisc. Almost immediately after surgery the sciatica is gone, and after a week I can walk around great and get back to work. Well worth the trip from Lake Charles to get an opinion from him. Thank you doc!
