Don't go there. The place wants all your personal information, like your Social Security number, your birth date, your employer's name and number, how long you've been on the job, etc. Then they said they had to have a copy of my driver's license!!
It's none of their business where you work or how long you've been there. It's none of their business what your SS number is, and they sure don't need a copy of my driver's license!! It's not going to help them treat me.
